The ZUS funeral benefit
Ladies and Gentlemen, we would like to inform you that from 1st March 2011, the funeral benefit paid by ZUS amounts to 4000,00 PLN. In the last period, i.e. from 1st December 2010 to 28st February 2011, the benefit amounted to 6406,16 PLN.
The person who incurred the costs of the funeral is entitled to the funeral benefit.
The employer, nursing home, municipal or district authority, legal person of the church or confessional association, is also entitled to the funeral benefit, if they have incurred the funeral costs.
Expiration of the claim for the funeral benefit:
The right to the funeral benefit expires if the application for it has not been submitted within 12 months after the death of the person, for whom the benefit should be paid.
Necessary documents:
The main documents necessary for receiving funeral benefit are:
►The application for the funeral benefit (application ZUS Z-12).
►Abridged copy of the death certificate.
►Original receipts of the incurred funeral costs and if the original receipts have been submitted to the bank – copies of the receipts, confirmed by the bank.
►Documents confirming kinship or affinity (abridged copies of the civil status acts or ID with the necessary data).
If the funeral has been organized at the cost of the state or a political or voluntary organization, it is necessary to submit the receipts and the certificate issued by the proper state administration organ or organization, stating that the person applying for the funeral benefit has indeed incurred the funeral costs.
